- Are your current doctors and preferred hospital in-network?
- Do you need referrals to see specialists?
- Is telehealth included, and at what cost?
- How are prescriptions covered, and is your regular medication on the formulary?
Coverage scope
- What services are explicitly excluded?
- Are pre-existing conditions covered immediately or subject to waiting periods?
- Does the plan cover maternity, mental health, or chronic condition management if relevant to you?
- Is the plan renewable, and under what circumstances could it change or end?
Practical logistics
- When does coverage actually start after enrolling?
- What is the process for filing a claim or appealing a denial?
- Who can you call with questions once you're enrolled?
